Mateusz Łoskot 79367e6284 Add Makefile for NMAKE and Visual C++ toolset. 
Add pre-digested config.h for Visual C++ on Windows
(courtesy of Martin Hofmann (tin-pot) fork of Discount).

Basic MSVC configuration allows to build libmarkdown and mkd2html utility.
