Fix memory leak in mkd_xml()
...
The Cstring |f| wasn't being DELETE()d before returning from mkd_xml(),
leading to a leak every time it was called.

when checking character classes move the utf-8 check in front of all of the other checks; some versions of MSC have a debug mode so if a character isn't either 7-bit ascii or EOF the character class check functions will abort the program. Like, um, yeah, way to creep those standards, people!

Replace the internal function __mkd_xml() with new published
...
functions `mkd_generatexml()` [write the xml'ed version of a
string to a file] and `mkd_xml()` [write the xml'ed version of
a string into `malloc()`ed memory]
Also move `mkd_text()` into mkdio.c, and put the new xml functions
into their own module.
